Application for New Passport in lieu of Lost Passport

Please note that if the existing passport is lost it is mandatory that the applicant should visit the Embassy in person in order to apply for a new passport. Please visit the official web site of the Embassy in order to book an appointment online before visiting the Embassy in person.

Please bring the following documents when you visit the Embassy for your appointment:

Required documents:

  1. The Form K (IM +35) – must be duly completed with the signature on page 2. The signature must not touch the cage. Instructions to fill the Passport Application (K35A)
  2. Lost Complaint Form
  3. 3 photos/passport photos should show the “direct face without spectacles”, both ears and the complete head must be visible (biometric & photo size 3.5cm * 4.5cm)
  4. Original Birth Certificate
  5. Original Marriage Certificate (Only for women)
  6. Original Sri Lankan National Identity Card (NIC). If you do not possess a Sri Lankan National Identity (NIC)card, please write a letter (English, Sinhalese, Tamil) stating the reason/s why you do not have a National ID card from Sri Lanka. – Sample Letter
  7. 2 copies from your Visa status in Germany (Aufenthaltserlaubnis). Please note that the visa must be valid for a minimum 1-3 months
  8. An affidavit signed by a notary explaining the circumstances of how, when and where the passport was lost. (Translation of the affidavit by a sworn translator in English if the document is in German) – You can sign an affidavit directly at the Embassy as well for which an additional consular fee of €64/- will be charged
  9. A Police Report (Lost Report) for your lost passport (together with an English translation of the letter) obtained from the relevant Police Station
  10. A letter from the city/district office (foreigners office) confirming that you do not have asylum and refugee status granted by the German Government (original and a translation of the letter in English are required.)
  11. If you are a holder of German Blue/ Grey passports, you have to surrender your Blue/Grey passport to the relevant German Authority and bring a confirmation from the Authority with an English Translation
  12. Relevant certificates/letters to confirm your profession/occupation if you need to include your profession in the passport 
     

Special Note: Fees will not be refunded under any circumstances, after issuance of a receipt.

IMPORTANT: SUBMISSION OF FORGED DOCUMENTS AND MAKING FALSE DECLARATIONS IS A CRIMINAL OFFENCE AS PER THE SRI LANKA LAW. SUCH CASES WILL BE REPORTED TO THE RELEVANT AUTHORITIES.

Fees:

  • New Passport Application Fee in lieu of a lost Passport with a Copy of previous Passport: €330.00
  • New Passport Application Fee in lieu of a lost Passport without a Copy of previous Passport: €418.00
