Alteration of the surname of a female applicant consequent to the marriage

Required Documents:

1. Form “O” (IM 37)
2. Original Birth Certificate with one (1) photocopy
3. Original marriage certificate with one (1) photocopy – If the marriage certificate is in German, an English translation is required.
4. Both Husband’s & Wife’s original passports with one (1) photocopy

IMPORTANT: SUBMISSION OF FORGED DOCUMENTS AND MAKING FALSE DECLARATIONS IS A CRIMINAL OFFENCE AS PER THE SRI LANKA LAW. SUCH CASES WILL BE REPORTED TO THE RELEVANT AUTHORITIES.
