Registration of Deaths
Declaration of Death has to be made by the Next of Kin (NoK) or a close relative or a friend or any other person who was with the deceased at the time of death. It is advised to contact the Consular Section of the Embassy personally or via consular.berlin@mfa.gov.lk if any further information are required.
Required documents for registering a death of Sri Lankan nationals who have passed away in Germany:
- Duly perfected application – (Please use Form B 11 for registration of a death occurred within 3 months, and Form B 15 for registration of a death occurred more than 3 months ago).
- Original Death Certificate issued by the relevant German Authorities with an English translation
- Coroner’s Report
- Passport and other available identification documents of the deceased, to prove Sri Lankan Citizenship
Special Note:
If the certificates and documents are to be returned by post, a self-addressed and stamped envelope should be submitted along with the application. However, the Embassy will not take any responsibility for documents lost in the post.
IMPORTANT: SUBMISSION OF FORGED CERTIFICATES OR ANY OTHER DOCUMENTS AND MAKING FALSE DECLARATIONS IS AN OFFENCE. SUCH CASES WILL BE REPORTED TO RELEVANT AUTHORITIES.

- Issuance of Certified Copy of a Death Certificate: €21/-
